The BY127 although it has a reasonable rating does fail, there was never any real proof but it was believed that spikes in the mains was the reason. Many of us would fit a 0.1uf 1kv cap between the anode and chassis, this obviated the 1000pf cap across it as the cathode was usually connected to the reservoir cap.
Personally I don't use BY127's or 1N4007's these days, I use 1N5408 Diodes.
