Regarding the amp with fuses in the cathode, I'd suspect the addition of a couple of Ohms in the circuit would be no worse than the tolerance of the cathode resistors, or if in a fixed bias set up a very slight adjustment of the bias level would compensate.
The World Design amp I did recently I added a fuse to each output stage on the common feeds to the output transformers.
Regarding the fuses in solid state amps I have seen audio modulating the supply lines after the fuse, the addition of a suitable decoupler offered a cure.
