Alas the result was that no one knew a source of these fuses.
The only suggestion I can offer is that these combined mains selectors and fuses were very common on VOX amplifiers produced during the 1960s. There are a good few companies on the internet that offer service on vintage VOX. Perhaps one of those might tell you what they do to replace a blown fuse.
